At Rotary Hospice, we understand that hospice care is about more than just medical needs – it’s about supporting the whole person and their loved ones. Our dedicated hospice social workers provide compassionate guidance, emotional support, and practical assistance to help residents and those in their circle navigate this journey with comfort and confidence.

Social Work Support

How Our Social Workers Can Help

Our hospice social workers are here to listen, guide, and advocate for you. They can assist with:

Emotional Support: Helping residents and families cope with feelings of grief, stress, and uncertainty. Offering guidance and mediation for families and loved ones facing difficult conversations and decisions.

Advance Care Planning: Assisting with decisions about care preferences, legal documents, and future planning.

Community Resources & Referrals: Connecting you with caregiver support services and other helpful resources within the hospice and the community.

Legacy Projects: Facilitating meaningful keepsakes such as hand molds and memory projects to honour loved ones.

Nobody Should Face

End-of-Life Alone

Hospice can be an emotional and challenging time, but you don’t have to face it alone. Our social workers are here to walk alongside you, offering comfort, knowledge, and support every step of the way.
If you have questions or would like to connect with a hospice social worker, please reach out to us at sw@rotaryhospice.ca. We’re here to help.

Bereavement Support is Available to Everyone

Grief is a deeply personal journey, and no one should have to walk it alone. Our bereavement programs are open to everyone in the community, whether their loved one was in our hospice care or not.

Steps for Support
This is a weekly walking group, where participants receive peer support from others who are grieving in the community. The walk is led by volunteers who have received hospice bereavement training. The walk takes place every Tuesday morning at 9:30am in Stratford. There is a short intake to complete prior to joining.

Bereavement Groups
We will be facilitating a three-week “Beginning with Grief” group that will run at various times throughout the year. Focusing on various losses, these groups will include time to share your grief experience and allow for the opportunity to make connections with others who are grieving. These groups are currently held in person at Rotary Hospice.

Quarterly Webinars
Our webinars take place in March, June, September, and November, and focus on a variety of experiences one may face while grieving and discuss supports for coping. These one-hour webinars take place virtually over Zoom in the evening, with a presentation and time for Q&A.

Honouring and Remembering Loved Ones

We believe in the importance of remembrance and coming together as a community to celebrate the lives of those we have lost.

Lights of Love: A heartfelt gathering in early winter, where families and friends gather to honour and remember their loved ones.
